The Knox County Community Health Center has received national recognition for its commitment to providing high-quality, patient-focused healthcare.
The health center recently earned Patient-Centered Medical Home (PCMH) Recognition from the National Committee for Quality Assurance (NCQA), one of the nation’s leading organizations dedicated to evaluating and improving healthcare quality.
The designation recognizes healthcare providers that emphasize coordinated, accessible and team-based care while placing patients at the center of the healthcare experience. The Patient-Centered Medical Home model encourages stronger relationships between patients and their clinical care teams, helping improve communication, preventive care and long-term health outcomes.
According to NCQA, practices that earn PCMH Recognition demonstrate excellence in several key areas, including patient-centered access and continuity, care management and support, care coordination, performance measurement, quality improvement and effective management of patient populations.

Research has shown the Patient-Centered Medical Home model can improve the quality of care, enhance the patient experience and increase staff satisfaction while helping reduce healthcare costs through better coordination of services.
Founded to improve healthcare delivery across the country, the National Committee for Quality Assurance is an independent, nonprofit organization that accredits and certifies healthcare organizations and recognizes providers demonstrating excellence in patient care. The organization is also responsible for developing HEDIS, one of the nation’s most widely used healthcare performance measurement systems.
Health center officials say earning the recognition reinforces their ongoing commitment to providing comprehensive, coordinated healthcare services for residents throughout Knox County while continuing to improve the quality of care available in the community.
